In order to start using our services, the first step would be for you to book an appointment. We will be collecting your E-mail Address, First Name, Last Name and Contact Number only.

After booking an appointment, you will be receiving an e-mail for your appointment confirmation and details. Other confirmations and clarifications may be addressed with you via phone call (on the contact number you have provided).

During your physical appointment with a Licensed Counselor, he/she will be collecting more information such as (but not limited to) :

• Arrest Details
• Medical Assessment/Certificate
• Other personal information

We use the information that we have collected to properly assess and evaluate your situation and help you get better to prevent further mishaps, accidents and violations.

We do not share your personal information with other companies, organizations, relatives or other individuals except in the following cases:

For external processing

We provide personal information to those in-charge of the Alcohol/Drugs Assessment and evaluation. Note that these are all Licensed Professionals and are accredited with Oklahoma License Reinstatement Services™.
